Nature of Services

I understand that Righteous Livity Works LLC provides wellness-focused services, including but not limited to:

1:1 health coaching
Fitness coaching and exercise guidance
Cooking classes and nutrition education
Group wellness classes and workshops
Corporate wellness programs
Faith-based wellness support

These services are educational and supportive in nature and may include lifestyle guidance, wellness planning, exercise instruction, nutrition education, and community activities.

Scope of Practice

I acknowledge that:

These services are not a substitute for medical care, diagnosis, or treatment by a licensed healthcare provider.
Coaches, instructors, and facilitators are not acting as physicians, dietitians, or licensed mental health professionals unless specifically disclosed.
I should always consult a qualified healthcare provider before beginning any new fitness, nutrition, or lifestyle program, particularly if I have a medical condition, am pregnant, nursing, or taking prescription medications.

Voluntary Participation

I agree to participate voluntarily in coaching sessions, classes, or programs. I understand that I may withdraw at any time.

Expectations & Limitations

My progress depends on my commitment to engage honestly, follow guidance, and implement lifestyle changes.
The Company does not guarantee specific outcomes (including weight loss, blood sugar control, detoxification, or fitness results).

Telehealth & Remote Services

If sessions are provided via telehealth (video, phone, or online platform):

I understand the risks and limitations of remote services.
I agree to use a secure, private space during sessions.
The Company is not liable for unauthorized access due to my use of unsecured technology.

Client Rights

As a client, I have the right to:

Be treated with dignity, respect, and cultural sensitivity.
Receive clear information about services, pricing, and expectations.
Confidentiality of my personal information, in compliance with applicable law.
Refuse or discontinue services at any time without retaliation.
Ask questions, voice concerns, and receive appropriate responses.
Request access to any documentation related to my wellness plan.

Client Responsibilities

I agree to:

Provide complete and accurate health and lifestyle information.
Inform my provider of changes in my health status.
Follow agreed-upon recommendations to the best of my ability.
Keep scheduled appointments or provide timely notice to reschedule.
Assume responsibility for any supplements, foods, or exercise practices undertaken.
Seek emergency care when needed from a licensed provider.

Health & Safety

Fitness Safety: I understand physical activity carries risks including injury, falls, cardiovascular events, and muscle strain. I agree to exercise within my limits and stop if I experience pain, dizziness, chest discomfort, or shortness of breath.
Nutrition & Allergies: Cooking classes and nutrition coaching may involve potential allergens. It is my responsibility to notify the Company of any allergies. I understand the Company cannot guarantee an allergen-free environment.
Faith-Based Content: Where spiritual or faith-based elements are integrated, I acknowledge these are optional and supportive, not therapeutic or prescriptive.

HIPAA Notice of Privacy Practices (Summary)

In compliance with the Health Insurance Portability and Accountability Act (HIPAA):

My identifiable health information is protected and used only for coordination of care, payment, business operations, or as required by law.
I have the right to access, review, and request corrections to my personal health records.
My data will not be disclosed without my consent, except as required by law (e.g., abuse reporting, public health).
I may file a complaint if I believe my privacy rights are violated.

Terms & Conditions

1. Services Provided

Righteous Livity Works LLC offers health and wellness services, including educational programs, lifestyle planning, coaching, and group/corporate offerings. Services are supportive, not medical care.

2. Eligibility

You must be at least 18 years old (or have written parental consent if under 18). By using our services, you affirm you meet eligibility requirements.

3. Medical Disclaimer

All content and services are informational only and not intended to diagnose, treat, or cure medical conditions. No provider–patient relationship is established.

4. User Responsibilities

You agree to:

Provide accurate information.
Maintain confidentiality of login credentials (for online services).
Use services lawfully and appropriately.

5. Payment & Refund Policy

Fees must be paid at booking or enrollment.
No refunds for completed services.
Refunds for unused services may be considered but are not guaranteed.

6. Cancellation & Rescheduling

Sessions must be canceled or rescheduled at least 24 hours in advance.
Late cancellations or no-shows may result in a fee.

7. Intellectual Property

All materials (guides, videos, PDFs, digital tools, presentations) remain the property of the Company. You may not copy, reproduce, or distribute without permission.

8. Privacy Policy

We collect and store only the personal information necessary to provide services, comply with law, and improve operations. By using services, you consent to this collection and use.

9. Limitation of Liability

To the fullest extent allowed by law, Righteous Livity Works LLC is not liable for injuries, damages, or losses arising from participation in its services. Use of services is at your own risk.

10. Indemnification

You agree to indemnify and hold harmless the Company, its employees, contractors, and affiliates against claims arising from your use of services or violation of these terms.

11. Modifications

We may update these Terms & Conditions at any time. Continued participation constitutes acceptance of updates.

12. Governing Law

This Agreement is governed by the laws of the State of Texas and applicable U.S. federal law.

Consent & Acknowledgment

By participating in any program or offering of Righteous Livity Works LLC, I confirm that:

I have read and understand the Consent, Rights, Responsibilities, HIPAA Notice, and Terms & Conditions.
I acknowledge that participation involves risks, and I voluntarily assume those risks.
I release and hold harmless Righteous Livity Works LLC, its owners, employees, affiliates, and instructors from any liability as outlined above.
